ECNL & NPL Waiver (2011-2007 ECNL & NPL Teams ONLY)

Players who reside in Kentucky must submit an online application for an Out of State Release. This is a requirement of the Kentucky Youth Soccer Association. https://www.kysoccer.net/forms/ EXCEPTION: U17-U19 ECNL & NPL teams NOT playing in the state league!
